Overview

Semax is a synthetic heptapeptide developed at the Institute of Molecular Genetics of the Russian Academy of Sciences as a stabilised analogue of the ACTH(4-10) fragment — the seven-amino-acid sequence responsible for neuroprotective and cognition-enhancing activities of adrenocorticotropic hormone, without its endocrine effects. Approved in Russia and Ukraine for clinical use in stroke, traumatic brain injury, and cognitive decline since 1995, Semax has been the subject of decades of rigorous investigation into neurotrophin upregulation, neuroprotection, and cognitive enhancement.

Unlike synthetic stimulants or nootropics that work by increasing neurotransmitter concentrations, Semax works at the genetic level — upregulating the expression of BDNF and NGF, the master regulatory proteins of neuronal survival, synaptic plasticity, and long-term potentiation. Mechanism of Action

Semax’s primary mechanism involves direct upregulation of brain-derived neurotrophic factor (BDNF) and nerve growth factor (NGF) — the master regulators of neuronal survival, synaptic plasticity, and long-term potentiation (LTP). BDNF elevation enhances the density and efficacy of dendritic spines (the physical sites of memory formation) and strengthens glutamatergic signalling in the hippocampus and prefrontal cortex.

Semax also increases expression of serotonin, dopamine, and norepinephrine receptors — producing anxiolytic and procognitive effects that are independent of classical neurotransmitter concentration changes. In ischaemia models, Semax reduces expression of pro-apoptotic genes and inflammatory mediators within the penumbra region, limiting neuronal death following vascular insult.
